Scott Weiland, the former frontman for Stone Temple Pilots and Velvet Revolver died of accidental "mixed drug toxicity,” it has been confirmed. The 48-year-old had a well documented struggle with drug abuse and was found dead on his tour bus in Minnesota on December 3rd.

A report from the Hennepin County Medical Examiner states that Weiland’s death was cause by a toxic mix of drugs and he had evidence of cocaine, alcohol and MDA in his system. The report also noted a history of cardiovascular disease, asthma and multi-substance dependence.

Weiland's body was found on his tour bus in Minneapolis on December 3rd at 8:27 p.m, according to the report. The frontman’s death was described as ‘accidental’. Weiland’s history of drug abuse was well documented and he had entered rehab on a number of occasions.

Following his death, Weiland’s ex-wife Mary Forsberg-Weiland published an open letter in Rolling Stone, speaking about the effects of his addiction on the couple’s two children, Noah, 15, and Lucy, 13.

"The truth is, like so many other kids, they lost their father years ago," she wrote. "What they truly lost on December 3rd was hope…You might ask, 'How were we to know? We read that he loved spending time with his children and that he'd been drug-free for years!’”.

"In reality, what you didn't want to acknowledge was a paranoid man who couldn't remember his own lyrics and who was only photographed with his children a handful of times in 15 years of fatherhood."

"Our hope for Scott has died, but there is still hope for others," she ended letter. "Let's choose to make this the first time we don't glorify this tragedy with talk of rock and roll and the demons that, by the way, don't have to come with it."
